For someone to be in the constant state of Paranoia. Paranoia is the thought processm, which is heavily influenced by anxiety and/or fear, often to the point of irrationality and delusion. Paranoid thinking typically includes persecutory beliefs concerning a perceived threat towards ones own self. Greek translation of paranoia simply means madness (para = outside; [nous] = mind). Historically, this characterization was used to describe any extreme delusional state.
Someone who is crazy and thinks everyone is always [out to get] them, take them down, or cause them to fail at everything and anything they [undertake].

Paranoid is the title of [Black Sabbaths] second album. It is often cited as one of the biggest milestones in heavy metal. It was released in 1970 and topped the British music charts. The album was originally going to be called [War Pigs], which is a war protest song and the opening number, but their record company made them change it due to the sensitivity about the Vietnam War.
2 singles came from this album. Paranoid and Iron Man, which would go on to become Black Sabbaths best known songs. Paranoid peaked at #61 and Iron Man #52. This album was also ranked 130 on [Rolling Stone] Magazines list of 500 greatest albums of all time.
