A person (typically male) or their actions, who has acted in such an obnoxious manner that the behavior alone reeks as an unsanitary twat would. Malicious intent and/or [malevolence] is a general added characteristic.
The most vile form of [Douche-Baggery]. In progression of severity of behavior: One could be a Douche, a Douche-Bag, [Douche-Nozzle], or most disgustingly Douche-Nozzle Backflow!
