Portopulmonary hypertension
Portopulmonary hypertension is pulmonary arterial hypertension associated with portal hypertension without other secondary causes.
Pulmonary hypertension occurs in patients with various conditions that involve portal hypertension with or without cirrhosis. Portopulmonary hypertension occurs less commonly than hepatopulmonary syndrome in patients with chronic liver disease (3.5 vs 12%).
Abnormalities in bone morphogenetic protein 9 (BMP9) signaling are linked to the development of pulmonary hypertension. BMP9 and BMP10 are produced in the liver and are ligands for the BMP2 receptor. Patients with portopulmonary hypertension have been noted to have a significantly reduced BMP9 level when compared to control patients with advanced liver disease. There was no difference in BMP10 levels when comparing patients with portopulmonary hypertension to patients with advanced liver disease without portopulmonary hypertension (1).
Presenting symptoms are dyspnea and fatigue. Chest pain and hemoptysis can also occur. Patients have physical findings and ECG abnormalities consistent with pulmonary hypertension and may develop evidence of cor pulmonale (elevated jugular venous pulse, edema). Tricuspid regurgitation is common.
The diagnosis is suspected based on echocardiography findings and confirmed by right heart catheterization.
Treatment of portopulmonary hypertension is the same as that of pulmonary arterial hypertension except that hepatotoxic drugs and anticoagulants should be avoided. Beta-blockers, frequently used in portal hypertension, should also be avoided in portopulmonary hypertension due to hemodynamic instability (2). Some patients benefit from vasodilator therapy. The underlying liver disease is a major determinant of outcome. Portopulmonary hypertension is a relative contraindication to liver transplantation because of increased morbidity and mortality from the procedure. However, in some patients who receive a transplant, particularly those with mild pulmonary hypertension, pulmonary hypertension regresses. Some centers consider transplantation in patients who have mean pulmonary arterial pressures -

The limited-slip differential (LSD) acts just like any other differential, distributing power from the driveshaft to the drive wheels. The main difference is that LSD involves a mechanism which limits the rotation speeds between the two axle shafts it spins. It is generally used on high performance cars to insure greater stability during turns and some sport utility vehicles.

Instead of being a mechanical device which regulates the torque distribution between two or more drive wheels, EDS uses the ABS sensors to detect wheelspin and brake each wheel individually , thus creating the illusion of torque distribution. Due to the stress it can create on the brakes by hard use, the system only works up to speeds of approximately 25 mph.

In order to do this, CBC is using the ABS sensors to determine each wheels rotational speed and will try to compensate the natural tendency of a vehicle to oversteer if braking during a turn.

With new genetic diagnostic and therapeutic capabilities come many controversies about how they should be used. For example, there are concerns that genetic information might be used improperly to discriminate (eg, by denying health insurance coverage or employment) against people with genetic risk factors for particular disorders. Issues include the privacy of a person’s own genetic information and the question of whether testing should be compulsory.
Prenatal screening for genetic abnormalities that cause serious disorders is widely supported; however, there is concern that screening could also be used to select for traits that are aesthetically desirable (eg, physical appearance, intelligence).
Cloning is highly controversial. Animal studies suggest cloning is much more likely than natural methods to result in defects that are lethal or cause serious health problems. Creating a human by cloning is widely seen as unethical, is illegal in most jurisdictions, and is technically difficult.
Inheritance of certain mitochondrial disorders has now been prevented by creating an embryo using biological material from 3 different people. Techniques are based on the fact that mitochondria are derived entirely from the mother and that mitochondria contain their own DNA separate from nuclear DNA.
In pronuclear transfer, a woman with a mitochondrial mutation has an egg fertilized in vitro with sperm from her partner. A donor egg from a woman with normal mitochondria is also fertilized with sperm from the same donor. Then the male and female pronuclei are removed from the first zygote and transplanted into the donor zygote after its nucleus has been removed. This fertilized embryo thus contains the sperm DNA from a male, the ovum DNA from the affected female, and the normal mitochondria (and their genome) from a second female, resulting in an embryo without mitochondrial disease.
Maternal spindle transfer is a similar procedure. In this case, an oocyte is removed from the affected woman. When the oocyte is in the metaphase II stage of cell division, the spindle–chromosome complex is removed and inserted into a healthy donor oocyte from which the nucleus has already been removed. This egg is then fertilized with sperm and implanted in the recipient -
