RE: Influenza vaccine
Based on recommendations by the World Health Organization and the Centers for Disease Control and Prevention (CDC), vaccines for influenza are modified annually to include the most prevalent strains (usually 2 strains of influenza A and 1 or 2 strains of influenza B). Heterophyiasis and related trematode infections
Heterophyiasis is infection with the intestinal fluke Heterophyes heterophyes, which is acquired by eating infected raw or undercooked fish from freshwater or brackish water.
Flukes are parasitic flatworms that infect various parts of the body (eg, blood vessels, gastrointestinal tract, lungs, liver) depending on the species.
Heterophyes heterophyes is endemic in the Far East, Middle East, and Egypt.
Heterophyiasis is acquired by eating infected raw or undercooked fish from freshwater or brackish water containing metacercariae (encysted stage). After ingestion, metacercariae excyst and attach to the mucosa of the small intestine. There, they develop into adults, growing to about 1.0 to 1.7 mm by 0.3 to 0.4 mm.
Infection with Metagonimus yokogawai, a related trematode, has been reported after eating raw or undercooked freshwater or brackish fish in the Far East, Siberia, Manchuria, the Balkan states, Israel, and Spain. Intestinal infection with Nanophyetus salmincola has been reported after ingestion of raw or undercooked salmon.
Adult flukes can cause abdominal pain and diarrhea.
Diagnosis of heterophyiasis is by finding eggs in the feces. The eggs of H. heterophyes are indistinguishable from those of M. yokogawai and similar to those of Clonorchis and Opisthorchis.
Treatment of heterophyiasis is with praziquantel 25 mg/kg orally 3 times a day for 1 day for H. heterophyes and M. yokogawai and 20 mg/kg orally 3 times a day for 1 day for N. salmincola.
Prevention involves not eating raw or undercooked fish that may contain these intestinal flukes. -
Alligator, crocodile, iguana, and venomous lizard bites
Venomous lizards, alligators and crocodiles, and iguanas are other reptiles that can cause clinically significant bites. Tetanus prophylaxis should be given (see table Tetanus Prophylaxis in Routine Wound Management).

Zika virusArthropod saliva composition varies considerably, and the lesions caused by bites vary from small papules to large ulcers with swelling and acute pain. Dermatitis may also occur. Most serious consequences result from secondary infection or hypersensitivity reactions, which can be fatal in sensitized people. Flea allergens may trigger respiratory allergy even without a bite in some people.
The location and pattern of wheals and lesions are sometimes diagnostic of the bite source. For example, blackfly bites are usually on the neck, ears, and face; flea bites may be numerous, mostly on the feet and legs; and bedbug bites often occur in linear patterns, most commonly on the torso. -

Distal radius fractures
Distal radius fractures usually result from a fall on an outstretched hand.

Most distal radius fractures are dorsally displaced or angulated (sometimes called Colles fractures); they are common, particularly among the elderly. Often, the ulnar styloid process is also fractured. Less often, volar displacement (called Smith fracture) occurs because the wrist was flexed during the injury.
